Parental Permission Form

In the interest of everyone's personal safety, students and volunteers alike, we need to have parental/legal guardian written permission on file each year for each child. 

The same form is used for both transportation and use of photographs meant to be used in multiple media settings. 

No personal identification will be used with photos other than a first name and where the photo was taken, with a description of the activity.
