Donation Forms

Set up a recurring donation to any of the three churches listed below. Fill out the necessary paperwork from the list below and return it to the bank listed on the form. All communications will be kept confidential between you and the bank and the church will not receive your account information.

A secure electronic form for the electronic transfer of account information and your electronic signature will be sent to you at the email you provide directly from the bank. Please allow two business days for the bank to respond. 

This recurring donation can be canceled at anytime by contacting the bank directly. This authority is to remain in full force and effect until the church or bank has received written or oral notification of its termination within two days of the next effective date.
